I'm so excited you're joining us at a Rhythm & Flow Studio event. To keep every participant safe and informed, we require every new attendee to read and sign the participant waiver below. This is a standard step for all movement, dance, and wellness events — and once you've signed, you're officially in!

dba Rhythm & Flow StudioAssumption of Risk, Release of Liability & Indemnification Agreement
1. Acknowledgement of Risk. I understand that participation in dance, movement, fitness, and wellness activities offered by Rhythm Goddess LLC dba Rhythm & Flow Studio involves inherent risks, including but not limited to muscle strain, sprains, falls, cardiovascular events, and other physical injuries. I voluntarily choose to participate with full knowledge of these risks.
2. Medical Fitness. I represent that I am in good physical condition and have no medical condition, injury, or disability that would prevent my safe participation. I agree to consult a physician prior to participating if I have any concerns.
3. Release & Waiver. I hereby release, waive, and discharge Rhythm Goddess LLC dba Rhythm & Flow Studio, its owners, instructors, employees, agents, and affiliates from any and all claims, demands, or causes of action arising from my participation, including those caused by negligence.
4. Indemnification. I agree to indemnify and hold harmless Rhythm Goddess LLC dba Rhythm & Flow Studio from any losses, liabilities, damages, or costs (including attorneys' fees) resulting from my participation.
5. Media Release. I grant Rhythm Goddess LLC dba Rhythm & Flow Studio permission to use photographs and video recordings taken during the event for promotional purposes without compensation.
6. Governing Law. This agreement shall be governed by the laws of the state in which the event is held. If any portion is deemed invalid, the remainder shall continue in full force.
7. Acknowledgement. I have read this waiver in full, understand its terms, and sign it voluntarily. I am at least 18 years of age (or a parent/guardian signing on behalf of a minor).
